This formula was first recorded in ‘Complete Works of (Zhang) Jing-yue’ (jing yue quan shu), by Zhang Jie-bing (a.k.a. Zhang Jing-yue), 1624. This is a compilation of Zhang’s works in 64 volumes. Together with detailed expositions on various aspects of TCM theory, diagnosis and treatment for the various branches of medicine, Zhang recorded many ancient and modern prescriptions as well as two volumes of his own formulas.
This is a strong Kidney Yang tonifying formula, which is similar to Fu Gui Ba Wei Wan (Rehmannia Eight Formula BP011), and may be used for much the same indications. The main difference is in the omission of the herbs with a draining nature: i.e. Poria (fu ling), Alisma (ze xie) and Paeonia (mu dan pi). This, together with the addition of five tonifying herbs, results in a formula with a powerful warm-tonifying action.
As a purely tonifying formula, it is constructed so that the Yang tonifying ingredients are balanced with Yin and Blood nourishing ingredients. The formula’s name (Right Returning/ Restoring Pill) refers to the right Kidney, which from ancient times has been regarded as the Yang Kidney, or the seat of the Kidney Yang.
In general, deficiency of the Kidney Essence manifests primarily as deficiency of the Kidney Yang or deficiency of the Kidney Yin. Kidney Yang deficiency may arise due to chronic illness, excessive work or activity with insufficient rest, excessive sexual activity, overuse of alcohol and ‘recreational’ drugs, or deficiency conditions of other Zang organs. In addition, there may have been an insufficient supply of innate Kidney Essence from the parents, which leads to the development of Kidney deficiency earlier in life.
It may also arise due to: the debilitating effects of repeated attacks of Cold or Damp pathogens or prolonged retention of the same; Yang deficiency of the Spleen; excessive consumption of Cold natured foods over a prolonged period; overuse of Cold natured herbs; or Kidney Yin deficiency, which may develop over time into Kidney Yang deficiency.
Clinically this formula is used when a relatively stronger warm-tonifying action is required and when disorders of fluid metabolism (due to Kidney Yang deficiency) are not present. In cases with marked fluid disorders (e.g. with oedema or Phlegm), the preferred formula to warm-tonify the Kidney Yang is Fu Gui Ba Wei Wan (Rehmannia Eight Formula BP011)
